But it’s still the same Pokémon it just changed its form. Are you saying if ditto transforms it’s no longer a ditto

It doesn't replace them.
The 'Chimera' is allowed because I have already explicitly permitted Doduo and Dodrio. 2 headed and 3-headed pokemon count as one pokemon. Doduo's existance means that siamese lions count as 1.
