I think its hard to define Spooky. I think if you lost brain nurons and became brain damaged, 'you' would be dead. I think if you could replace them, the new brain damaged 'you' would then die and a new, 3rd different 'you' would exist

I believe in minimizing the measured distance between instanced changes when possible
I obviously cannot stop changes from occuring, but I will not actively cause large ones unnecessarily
there is no point to life either way
scenario, you get shot in the head by an Indian, you lose 2% brain matter but survive
the most anyone around you notices has changed about you is you now like smooth foods, your self-analysis seems to check out as well, no new defects , etc.

I would fundementally not accept that my self-analysis checking out is accurate. I do not think I can fundementally know the state of my mind and how consistant it was to its pre damaged state

Regardless of my evaluation, it fundementally cannot be trusted is my point. Even if per my own evaluation I am 100% equivalently the same, I cannot know or trust that analysis to be true.

I would choose the procedure with the largest chance of overall survival. Brain cancer is a death sentence if you do not fix it, so it coming back would be not ideal
we can play with the odds, but that's a distraction

I think I can only quanitify my governing principal as "when possible, minimize risk of death, both in an identity and in a literal sense"
and I try to determine what that actually means as situations arise
