The main villain framed him for a crime and was sentenced to life in prison, where they used experimental drugs on him that caused him to gain feminine qualities

Dysphoria is caused by biological conditions that cause visbal physical differences from cis people

Presumably whatever drugs they gave the person caused dysphoria along with everything else that happened in the anime
